Turkey Tail — The Most Studied, Least Accessible Mushroom

We looked at three things for each mushroom: the number of published human randomised controlled trials, the size and duration of those trials, and whether independent research groups had reached broadly similar conclusions. A mushroom rated "Strong" here, such as Turkey Tail's PSK, has multiple sizeable trials and at least one large systematic review or meta-analysis behind it. "Moderate" reflects a small number of decent-quality human trials showing a consistent effect, without the volume needed for a confident meta-analysis. "Preliminary" means the human evidence is limited to one or two small trials, or is missing entirely, with support instead resting on cell studies, animal models, or centuries of traditional use that have not yet been tested rigorously in people.

None of these ratings are a verdict on whether a mushroom "works." Traditional use over centuries carries its own kind of evidence, and mechanistic plausibility from cell studies is not nothing.
